Job Description

Reporting to the Clinical Leader or Clinical Manager and under the direction of the practice providers, the Clinical Practice Nurse 1 delivers high quality patient care including assessment, treatment, planning, and care evaluation. The Clinical Practice Nurse 1 uses knowledge of the principles of growth and development over the life span, assesses clinical data reflective of the patient's health status and interprets information needed to identify each patient’s requirements relative to his/her age specific needs. The Clinical Practice Nurse 1 engages in caring relationships with patients, families, and caregivers, works collaboratively with the health care team, and uses sound judgments while performing nursing interventions. Minimum requirements: Associate Degree or Diploma in Nursing from an accredited School of Nursing. Licensure required: New Hampshire Board of Nursing license. Certification required: Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers. Demonstrated ability to effectively and positively interact with a multi-disciplinary staff, patients, and others. Responsibilities include establishing priorities in organizing daily activities and maintaining efficient patient flow; triaging patient phone calls following established guidelines and prescribed format for identifying patient's health problems or concerns, and providing quality patient care; administering prescribed medications; providing education to patients in response to identified patient needs; documenting health information, interventions, and outcomes into the patient electronic medical record; delegating patient care activities using the five rights: right task, person, direction, circumstance, and supervision (ensuring the task has been completed satisfactorily and documentation is complete) and providing feedback to delegatee and manager; applying principles of purposeful, organized thinking based on the nursing process to make sound clinical judgment and decisions; completing practice specific clinical skill competencies on an annual basis. Physical and work requirements include light material handling, frequent lifting up to 10 pounds, occasional lifting up to 20 pounds, repetitive motion, hearing, speaking, bending, fine motor skills, reaching, sitting, standing, walking, occasional climbing, kneeling, smelling, squatting, and specific vision abilities. Exposure to bloodborne pathogens, bodily fluids, airborne pathogens, electrical hazards, and toxic or caustic chemicals. Noise level usually quiet.
